Purpose: The
purpose of this formative assessment is to provide information about students’
understanding of different shapes. The results from this assessment will assist
the teacher in deciding where to move next in the shapes unit and decide which
areas the students could possibly need more review. Another purpose of this assessment is to give
students the ability to realize what areas of the unit they understand well and
what areas they need to work on more.
Standards/Benchmarks:
G.GS.02.01 Identify, describe, and
compare familiar two-dimensional and three-dimensional shapes, such as
triangles, rectangles, squares, circles, semi-circles, spheres, and rectangular
prisms.
G.GS.02.02 Explore and predict the
results of putting together and taking apart two-dimensional and
three-dimensional shapes.
G.GS.02.04 Distinguish between
curves and straight lines and between curved surfaces and flat surfaces.
G.SR.02.05 Classify familiar plane
and solid objects, e.g., square, rectangle, rhombus, cube, pyramid, prism,
cone, cylinder, and sphere, by common attributes such as shape, size, color,
roundness or number of corners and explain which attributes are being used for
classification
G.TR.02.06 Recognize that shapes
that have been slid, turned, or flipped are the same shape, e.g., a square
rotated is still a square.

Directions: Read Aloud
Today we are going to see what we know about
shapes! We have been learning all about
the different kinds of shapes and what they look like. We will take a test and
then use our tests to figure out what we know about shapes, and what we still
need to work on.
There are four parts to this test.
The first
part is fill-in-the-blank. You must
choose the answer that will make the most sense when placed in the blank. Don’t cross out the words after you have used
them because they may be used more than once.
Also, remember that not all words will be used.
The second part is a short section of True or
False questions. Just
circle true or false beneath the question.
The third section is multiple choice. Circle the letter that has the words that
will best answer the question. Remember
to read all choices before deciding.
The fourth part is a
matching section. In this section, the
choices will only be used once so you may cross out the choices after you have
used them. However, remember that not
all choices will be used.
Throughout
the test make sure you read each question a few times before deciding on an
answer. Each question is worth 2 points.
Remember to put your name and date at the top of the first page of your test.
Take your time second graders! You have all of math class to work on this test.
If you need help reading a question raise your hand and I will help you. If you have any other questions during the
test raise your hand and I will come to you.
Make sure to go back and check all
of your answers before you turn in your papers on my desk. When you are
finished with the test you may read a book of your choice until everyone is
done. There is no talking until all

Interpreting the results – have overheads of the charts available to

Teacher - Read aloud after
passing out chart 1: I
have finished grading all of your shapes tests from last week. We are now going to use our shapes tests to
figure out what we know about shapes, and what we need to work on more. We are going to use two charts to figure this
out. I have already passed out the first
chart. Go through your test and mark an
X next to the number if you got that one wrong, and an O next to the number if
you got that one right (demonstrate on overhead).
Teacher – Read aloud after
passing out chart 2: Now
that we have filled out our first chart, we are going to use it to fill out the
next chart. In this chart I have included your learning targets. These are the things that I said you would
know or be able to do by the end of our shapes unit.
To
fill this chart out, first, use chart 1 to put X’s by wrong answers and O’s by
right answers on the blanks next to the numbers. Then for each target, add up the total right
and total wrong and write the number in the box next to it. For example, on the first box, if I got
question 4 wrong, but I got 5, and 15-20 right, I would write a 7 in the total
right box and a 1 in the total wrong box (show this on overhead).
After
you have filled this out, decide which targets you know, and which you need
more help on. If you got all of the
questions right in the learning target, then you know it. Check the box that says “I know this”. If you got one wrong on any of the targets,
then you need to check the box that says “I need more help”.
After
you have filled out chart 2, take a look at what you know, and what you need
more help on.
